APPLICATION PACKET

The 2012 Scholarship Application Packet is now available for download.  Please note that the form can be filled out on your computer and then printed for submission. 

The Jackie Spellman Scholarship is available to U.S. Citizens who are high school seniors, community college students, four-year university students or graduate school students who are leukemia or lymphoma survivors, further defined as those individuals diagnosed with leukemia or lymphoma, as well as those individuals whose immediate family members have had or have leukemia or lymphoma while part of the same household.  Applicants must meet all criteria established within the application process. 

WINNERS OF THE SCHOLARSHIPS WILL BE NOTIFIED IN MID-JULY 2012.  ONLY THE WINNERS WILL BE NOTIFIED.

*Additionally, each scholarship awarded to a recipient is awarded for undergraduate and graduate studies at an accredited college or university, as defined in section 170(b)(1)(A)(ii).  The scholarship funds will be paid directly to the college or university for tuition and fees.  Any unused funds must be distributed back to the Foundation.
